Follow the steps below to access e-forms.  E-forms are only available to F-1/J-1 students and scholars. 

Option 1

1. Search: globalspartans.leela-thaimassage.com
2. Click “Log in” with your 菠菜网lol正规平台 credentials
3. Click on the category to search for e-forms.

Option 2 

(If you have issues with logging in one.leela-thaimassage.com or DUO authentication, do not use this option). 

1. Log in to one.leela-thaimassage.com using your credentials
2. Search for “iSpartan” app
3. Click on the category to search for e-forms.
